General Information

The Euclid Municipal Court address is required on all forms.
Plaintiff’s Attorney’s name and address should appear on all forms.
Judgment Debtor’s name and address should appear on all forms.
All Affidavits must be notarized.
If service by certified mail is requested, an additional $10.00 fee must be remitted.
The United States Post Office requires all garnishment orders to include the Judgment Debtor’s Social Security Number and to be served by Certified Mail.
US Bank requires all attachments to be served by Certified Mail.
KeyBank requires $1.00 processing fee to be paid by check.
Please include additional postage paid envelopes for any requested receipts and or Garnishment disbursements.
